The Care Transitions Education Project (CTEP) is an innovative frontline workforce development strategy that increases nurses’ knowledge and skills for executing effective patient care transitions and lets nurses practice them during interactive learning activities and experiences.

CTEP educates nurses from across care settings and roles together, increasing their ability to communicate and collaborate to achieve common patient-centered goals, including reducing avoidable hospital re-admissions.

Nurses, nursing students and cross continuum team members who participate in CTEP gain the competencies they need to successfully implement care transitions quality improvements.
