The Care Transitions Education Project (CTEP) is an innovative frontline workforce development strategy that increases nurses’ and other patient care professionals’ knowledge and skills for executing effective patient care transitions.

CTEP educates learners from across care settings, employing interactive learning activities and experiences to build their capacity to achieve common patient-centered goals, including reducing avoidable hospital re-admissions.

Learners gain the competencies they need to improve their own practice and implement care transitions quality improvements.
